An application for the issue of a remote sensing license shall be submitted to the body responsible for the mapping support for defense. An application for a remote sensing license shall contain, in addition to the data listed in the General Administrative Order Act: a) person making the request, telephone and fax number, e-mail address; (b) location of remote sensing, landing and landing, the planned flight path on the map or on a copy of the map in triplicate; (c) planned period of implementation; (d) the type of aircraft, the name of the operator; (e) the name, address of the executor of remote sensing; (f) the number of the aeronautical remote sensing activity license issued under the Aviation Act; (g) the purpose of remote sensing; (h) technical parameters for remote sensing, remote sensing devices; (i) the pre-authorization of telecommunication transmission of data, images and signals received or recorded by the recording equipment during the remote sensing process; (j) have the right to disclose whether the financial resources used for remote sensing contain public funds in whole or in part. An application for an authorization as provided for in paragraph 2 may also be made by the use of another service, in accordance with the Government Decree on the Customer Gateway or Regulated Services of the Electronic Administration and Services to be Provided by the State.
