This Regulation of the Minister of Agriculture and Rural Development establishes the new list of animal infectious diseases being subject to notification in the European Union (specified in Annex 1), the form of report of the district veterinarian on the suspection of animal infectious disease (Annex 2), the form of report of the provincial veterinarian on the suspection of animal infectious disease (Annex 3), and specifies the scope of information included in the notification on animal infectious disease (Annex 4).
